Was entangled in a turn pulley for endless haulage rope leading from the counter balance pulley into the engine house underground. How he got there is unknown. He had been sent to tighten the brake but before completing it allowed the engine to be stared the engineman finding he had lost control and that the brake would not act called to everyone to leave the engine house which they did including Richards.
