- Richards was using a Sylvester appliance in order to withdraw a prop which had been uncapped from the roof in the waste, but was fast at the foot. After attaching a chain to the prop in the waste, he was said to have imprudently attached the other end of the chain to a sprag which was supporting some top coal at the face. This sprag gave way when using the Sylvester and caused a large piece of coal 6 feet in length to fall upon the deceased. He expired on reaching the surface from the effects of the injuries received.
