- He was in charge of an Electric (Motor) haulage engine underground in the N.W. District Big Vein. It appears that the wires for the electric lighting (5 lamps in series) were so placed that they frequently got damaged by horse passing along the road and that they were frequently jointed by and everyone and these joints were not covered or insulated. Rhead appears to have gone to examine the wires which had again been disturbed by a horse and in turning a slack one out of the way he caught the bare part and received a severe shock from which he died. Unfortunately there was no one present who understood First Aid or there seems every reason to think his life would have been saved. The electrician appears to have been very slack about his work and was also found so on one or two previous occasions.
