Deceased had been lowered on the top of the cage to the middle lift of the pump, and while he was attending to the pump a change of engineman took place the out going on duty told the other where deceased was. In a short time deceased signalled to be lowered to the bottom and the engine was put in motion but the engineman forgot temporarily the position of the cage and lowered it on to the bottom with such force. Deceased was knocked off by the bridle chains and his skull fractured. The engineman had worked the engine for 24 years without mishap. The engine was in good condition. He died in three days.
