Details of Event
Deceased and another man were taking out bars for the purpose of making height in a roadway. They were instructed to set temporary supports before loosening any timber. They knocked out a prop then attempted to withdraw a bar by means of a Sylvester apparatus. Failing to do this, deceased was using his, pick to loosen the end of the bar when the roof collapsed and killed him. No temporary supports appear to have been set.
