While removing a bar machine coal cutter, after it had reached the end of its cut the machine was started with the cutter bar out of the seam, which is contrary to the maker’s instructions. The teeth caught in the man’s clothing, and drew him under cutting and mangling him so severely that he died in a very short time after he was extricated.
