- Deceased was unloading a tram of rubbish on the side of a heading when he was caught by another tram of rubbish which came from higher up the heading and crushed between the two. The runaway tram was being hauled up the heading by a chain attached to a haulage rope and had a barhook behind. The tram left the rails and the rider took off the barhook in order to lift the hind end on the rails. In lifting the front end on the rails the chain somehow became detached and the tram ran back. The rider stated that he put two sprags in the wheels before removing the barhook. Died Feb. 28th, 1904.
