The accident was caused by the snapping of a 4in x ¾ in mild steel hitching plate or draw bar across a rivet-hole, at the tail end of the first tram in a train of nine loaded ones as the journey was getting over a brow having an inclination of 20 degrees, at the mouth of the main haulage slant. These men with many others were walking up the slant at the end of the shift, when they were run over and killed. 6 killed. Five others were injured. No barhook used.
