Deceased and a repairer were ascending the shaft and when 10 yd up the rage tilted on one side and deceased was thrown out. The repairer's collar-bone was fractured by his being thrown violently on to the floor of the cage. After the accident four feet of one of the T head rail guides was found broken off through it was thought the fish-plate breaking and liberating the guide end at which the cage caught. The shaft and guides were examined a few hours previously and found in good condition.
