Deceased was working on a main haulage brow when he noticed a set of two full tubs had left the rails. In attempting to reach the electric signal wires for the purpose of stopping the rope, his foot slipped and he fell in front of the set. He was crushed by the tub and received injuries from which he died on 19th June, 1913.
