- Deceased had holed in the middle dirt of the 6 feet seam to a depth of nearly 3 feet and for a length of 8.5 feet, and had a loose end on the left side of his working. He had set one short sprag upon the bottom coal near the centre, but the top coal and dirt pushed it out and it fell upon him killing him instantly. He should have had at least one additional sprag near the loose end.
