- Whilst walking towards the shaft up an engine plane having an inclination of 1 in 8, he was struck by a run of empty tubs and sustained a compound fracture of the left ankle. His leg was amputated a week later, and he died in hospital on the 19th January, 1911. The plane has a single line, of rails. It is 10 feet 7 inches wide and is provided with refuge holes. It is the regular travelling road, but travelling is prohibited while the rope is in motion. Deceased should have waited until the rope had been stopped.
