When ascending the shaft with 15 other men at the end of the shift, he either fainted or had a fit and fell on to the bottom of the cage and rolled out at the side of it into the shaft when about 540 feet from the bottom. The ends of the cage were open and the sides were open and the sides were only protected by two bars one 1 foot 10 inches and the other 2 feet 8 inches from the floor.
