Details of Event
The steam pipes at the top of the shaft required lifting. The capstan engine was otherwise engaged so the decease and another man attached the winding rope to the pipes with a lashing chain. They were in the cradle hanging just below the sill and when the winding rope was tightened the lashing chain broke and deceased was so badly crushed against the sill that he died in two hours.
