- A fall of roof had occurred in an endless rope haulage dip. A fireman sent Prince to the top of the dip to make things safe for men to work at the fall without instructing him what to do. Prince protected the empty road by a rail but did nothing on the fall side. Whilst he and others were working at the fall two full tubs were accidentally pushed into the dip and they ran down wild and caught Prince. The fireman should himself have seen that both lines of rails were protected.
