He was lowering an empty truck to end of wharf and as the brake lever was in front and on the wall side he rode on the front buffer of the truck and lifted the lever with his foot. He lifted it into catch at top of guard, and was unable to lift it out again so as to apply the brake to stop the truck. The truck ran past the wharf and he jumped off on to an old ash-tip lost his footing and rolled against the truck The axle box of the near wheel struck him.
