- While he was working in a place at the bottom of the quarry, directly under a steeply inclined aerial ropeway, a loaded wagon, which was suspended from the rope was being raised from a ledge some distance from and above him. Owing to a clamp, called a stop-jack on the stationary rope failing to hold in position the traveller or carriage the loaded wagon was suddenly and rapidly carried down the ropeway. At a point near to where he was working failing to hold the attachments between the hauling rope and the loaded wagon came into violent collision with the traveller. The force of the impact sheared off two bolts which connected the hauling rope swivel to the load. As the detached wagon shot forward its rock contents fell out. In trying to escape injury, he ran in the path of the falling material and some of the rocks fell on and killed him.
