- The Wellington pit, 700 feet deep is traversed by two three-decked cages each cage carrying three tubs. The deceased at the end of his shift got into the upper deck, which id 5 feet 8 inches high of the cage at the pit bottom with 5 other persons when the cage was being raised to enable persons to get into the middle deck a safety lamp, weighing 3.5lb fell from the surface and struck the top of the arched cover of the cage which was made steel, at a point with the underside which deceased's head was in contact. He was rendered unconscious and died the same day in the Infirmary. There was practically no dent in the cover caused by the blow. The lamp fell in some unexplained way from the belt of a miner who was stepping on the flat sheet on the surface ready to get into the top deck of the cage there then being loaded with men.
