Was engaged repairing downbrow. When some tubs of dirt were being drawn by endless rope, one of them left the rails. The haulage was stopped and when Penketh was lifting on the tub, the haulage restarted unawares, and he got jammed and fatally injured between tub and roof. It was thought a fall of roof might have caused the signal to ring.
