- Deceased was foreman erector in the employ of an engineering firm who were installing a new drum on a winding engine. He was crushed about the head by a steel girder 20 feet long, which was being raised into position to form the drum brake. It was being lifted by a chain of 3/8 inch iron with two 1/4 inch links in it. The lower end of the girder was shaped castings attached was resting upon part of the rim of the too old drum so that the chain was not bearing the whole load. Deceased stood between the girder and the wall of the engine house to adjust a bolt in the casting at the top end. While so doing the chain broke and the girder fell and crushed his head against the wall. There was nothing to account for the chain breaking unless the lower end of the girder slipped off its resting place, in which case it would drop a distance of some 8 inches.
