- He was killed in a sinking pit by a water tank descending unexpectedly upon him. It appears that a piece of string which worked the indicator of the winding engine had been broken, and the engineman depended upon a chalk mark on the rope, hence made a mistake in the distance. He should have reported the breakage of the indicator to the mechanical engineer and should not have worked the engine until it had been repaired and adjusted. Ht was prosecuted for a breach of Special Rule 109 and was convicted.
