- Deceased was filling a tub on a longwall face in the Shallow Seam when a rate of coal and bannocking dirt 9 feet brig, 5 feet deep and of thickness tapering from 18 inches at top to nothing at the bottom, fell suddenly from the face and crushed his head against a tree so severely that he died almost immediately. The rate came off at a black slip or facing which was not visible before the accident.
