Deceased and another man were hewing in a seam 1 foot 11 inches thick, worked longwall. A roll and a small hitch was running close to the righthand side it the gateway. As deceased was starting to hew in front of the gateway a large stone from the roof of grey stone, measuring 5 feet 6 inches long, 3 feet 6 inches wide by 15 inches thick, came away canting out one prop, and crushing him to death. The stone came from several slips in the roof, no signs of which were apparent before the fall.
