Details of Event
A piece of coal resting against an upthrow fault suddenly slipped down the inclined floor, and crushed the deceased against the pack wall, killing him instantly. A slip at right angles to the fault was exposed afterwards. The presence of the fault was known, and therefore the piece of coal should either have been secured by means of a sprag.
