Deceased struck one blow with an axe at a brattice strip nailed to the leg of a bond of timber. The blow caused the bond and another one set touching it to collapse, and about two tons of roof shale fell from two slips. Deceased was struck on the back by a piece of falling timber and received such severe internal injuries that he died in a few minutes.
