- The accident occurred in a level breasting which was being driven alongside an air-head in the Ten Feet seam, which has an inclination of 42 degrees. The seam was a strong bass roof. Chocks are set 2.5 feet apart on each side of the wagon road, a dirt pack being built on the head side. The timber one the lower side is afterwards withdrawn. Deceased was stooping to clear some small pieces of dirt off the wagon rails when suddenly the roof in the waste fell and displaced two newly set chocks, and the fall extended across the road, knocked deceased down, and buried him. He was dead when extricated. The fall came from a slip in the roof extending from the direction of the waste across the roadway.
