- An empty tub became derailed on an endless rope haulage road, and getting fast caused a straw on the ropes which pulled out the terminal pulley-frame along with the timber to which it was attached. The consequent fall of roof buried deceased. The terminal pulley-frame was not anchored, and owing to the method by which the framing was secured, the sudden strain on the rope and frame pulled out the timber which kept the frame in position, and also acted as the roof support.
