- Deceased was putting a tub into the cage at shaft bottom when the back of his right thumb was caught and cut by something said to be a small piece of brick or plaster which fell down the shaft. The piece probably fell from the shaft side at the old Thick Coal inset above, which was built up and plastered on account of fire, and at which there was some crush and movement occurring. Deceased worked the following day, but blood poisoning supervened, and he died on 11th November.
