Details of Event
Whilst acting as knocker-off of the tail rope on the main haulage road at the shaft siding, deceased's head was caught between the last tub of the full set and a low girder as the set was moved inbye after having come to rest. Deceased had been warned that the set was going to be run back. He died four days later.
