- After a Great Northern Railway train had passed up the sidings with empty wagons, deceased coupled six partly filled wagons to the colliery locomotive, and stepped back while these were pulled out. He stood in the four-foot way of the Great Northern Railway watching what was going on, when the Great Northern Railway locomotive and some wagons returned and ran over him. There was considerable noise from adjacent cleaning machinery. The Great Northern Railway driver did not use the whistle, and no look-out was kept.
