- At the end of the shift deceased was ascending in the cage with seven other men, when he suddenly fell forward into the shaft and dropped a distance of 100 yards. He was the last man to get into the cage and so stood at the end and when the cage started he had his lamp in one hand and was holding the bar with his left hand. His fall was apparently due to his having taken a fainting fit.
