Details of Event
A belt was slipping on a pulley and Oldham was trying to remedy the defect. He went up a ladder to turn a fly wheel and his clothing was caught by a revolving shaft. He was whirled round and fatally injured. It was not expected that anyone would be in that place whilst the machinery was in motion.
