- Along with another man he was making an additional refuge hole on a Main Haulage road in order to comply with the new requirements, the existing holes being 15 yards apart. At the point where the hole was being made the roof half been closely timbered for years, hut while wedging down a piece of stone a sudden crump or weighting came on and canted out three bars. Probably in wedging down the stone the end support of one of the bars was weakened. The place was apparently well secured, but it would have been safer to have given additional support to the bars by setting another running bar near the ends.
