Owing to a large increase of water chests had to be requisitioned to raise it from the shaft bottom and were to be emptied on arriving at the top automatically by lever arrangements. When the first chest arrived deceased and the oversman got into it in order to fix the position at which the lever was to come into operation to empty the chest and while thus engaged chest was suddenly raised and he was crushed against the fence at the top. The emptying of the chest had taken the weight off and the brake not being on the engine moved.
