- He rushed up to get on the last cage of men going down the cage with 10 men including deceased stopped at the 500-yard level for three of the men to get off for which purpose a drawbridge was used. While the cage was stationary there and after the drawbridge had been drawn up O'Donald fell out of the cage and fell 210 yards to the bottom. No one was able to account for him falling out. There were hand-bars in the cage to hold on by but not fences at the ends of the cage.
