- The pump attendant, contrary to orders, was riding from the 60 Fathom level, standing on the edge of the kibble on its reaching being 30 fathom level, Nicholls stepped out from the hanging side of the shaft and attempted to get on to the kibble behind the pumpman. He either missed his mark, or on stepping on to the kibble it tilted over with the result that he fell down the shaft and was killed. The occurrence was due to the man's disregard of orders given by the Manager and by the Pitman and to the dislike on the part of the latter to report to the Manager when he found the men disobeying the orders he had given then. Nicholls was working between the 25 and 30-Fathoms Levels and riding on the kibble was allowed from the 25-Fathom level as the shaft to that point was vertical. Instead of climbing to the 25-Fathom level he chose to step down to the 30 fathom Level and to attempt to jump on to the moving kibble.
