- Deceased was run over by runaway hutches whilst walking up a haulage road. These hutches formed part of a rake of nine, one of which had become derailed, the bogieman and two benchers were after uncoupling the five near hutches to lift it on, when those behind ran down the road and caused the accident. There was a jock behind the hutches, but it is probable as it had two prongs it slid down the rope. The men replacing the hutch on the rails were much to blame for not seeing those behind were secured, so that they could not possibly run down the road.
