Deceased, with two other workmen, was engaged renewing slides in the shaft, and had to work on the cage top. He was coming off the cage top to the cage bottom, and his feet slipped, whereby he fell into the shaft, a distance of 18 fathoms. The position at the time was opposite the door heads of an upper seam, and he got past the opening.
