He went up a ladder to the top of some screens in the course of construction, in the dark, and was passing across the stage when he fell through a hole on to a coal shoot 16 feet below and was killed on the spot. He should have taken a light hole with him and the hole should have been covered at night it had been uncovered by some contractors removing a flat so that some blocks could be attached to a girder.
