He was examining the spear rods and pumps in the shaft and got on to a collar to localise a ‘rattle’ which seemed to indicate that a rubber was loose and while he was listening the safety block on the spear rods came down upon him and crushed him. The pumps were working three strokes a minute and he evidently forgot that he was in danger from the descending stop block.
