Deceased, and his under manager had been examining the workings of a seam in the shaft, and on arriving back they signalled to he raised to the surface, when all was in readiness they stepped on to the cage and gave the signal and just then deceased saw that the gate, which was a sibling one, facing the opening was not closed down, and he put out his right foot to press it when the cage was suddenly raised, and losing his balance he fell off the cage into the shaft. The under manager made an attempt to save him, and caught his jacket, but he was unable to hold him.
