Ladders were being fixed in a blind shaft communicating two seams and while deceased was ascending the ladders already in with a plank on his shoulder another plank fell from above and struck him The plank which fell was being put in as a bunton on which to take a landing and the person fixing it mis-hit a nail and the bunton slipped from its position and went down the shaft.
