- Deceased and two other men were working a Hurd Electric Bar coal cutting machine and when doing so found the roof at the face under which they had to pass very bad and, therefore, decided to get the machine away to the gate road as quickly as possible. As a rule before the machine is moved along the face the cutters are taken out which takes from 15 to twenty minutes to do the men decided, however, as there had already been a fall and the roof was weighting to get the machine away without doing this. The deceased was near the sleeve end of the bar cotter between the machine and the face racking the bar away from the face as the machine was being hauled along when he stumbled and was caught by the bar and so badly cut that he was dead when got out.
