- The cages at the shaft at which this accident occurred have two decks and there are two stages the top decks had no cover but the floors of these decks acted as covers overhead when men were being raised or lowered. By some oversight, a clamp and chain weighing 11lbs. were sent up the shaft with the full tubs. The banksman saw them knocked down the shaft by striking the flat sheet staging just as the cage reached the surface. The deceased was at the time pushing the empty tubs out of the top deck of the descending cage when the clamp fell upon him and fractured his skull.
