- Deceased and a gang of men working under his instructions were taking down the ends of eight old guide ropes from the headgear. When the guides were last changed these ends were cut off above the glands and allowed to remain on the framing. Seven of these ends had been safely got down, and they were engaged on the eighth, when it unexpectedly started running, and the end struck deceased who was standing on a horizontal crosspiece between the backstays and threw him on to the roof of a shed below. The precaution of fastening a rope to the upper end of the piece of guide rope had not been taken in this case though it is said to have been in getting the others down. It is said that considerable difficulty was experienced getting the first seven down and that consequently they did not expect the eighth to move so easily.
