- He had been sending an empty train off on the running-on side and was, on his way back to knock off the catch on the other side of the shaft. He passed the onsetter who was taking a locker out of a tub and had his hack turned. Before the onsetter had taken the locker out he heard a thud and found that deceased had fallen down the shaft, a distance of 155 feet. There was not time for deceased to go round the proper way, and it is conjectured that he was crossing the shaft on the cage landing beams and slipped through. This was a violation of General Regulation 22. Owing to the fences protecting the shaft it was considered impossible for anyone to slip down the shaft accidentally.
