- A wheelbarrow filled with sand was being raised from a quarry 26 feet in depth by means of a ropes passing over a pulley carried on shear legs placed near the lip of a nearly vertical face. The face suddenly slipped in along a length of 30 feet and to a maximum width in centre of 6 feet, tailing out at both ends and bottom. Deceased, who was wheeling sand forward to this bottom of this hoist, was caught and buried by the falling sand, his neck being broken. This foreman had examined the lip in this morning and saw no crack. The surface consisted of soil and gravel for a depth of 2 feet back from this lip, which probably hid the crack.
