Deceased was knocking out a post with a hammer in a waste, in the Twist coal seam. A slab of the shale roof, 8 inches thick, fell and crushed his head on a heap of debris lying round the post, causing instant death. The height of the working was only 3 feet, and it was considered safe to draw the post without a chain. The use of a ringer and chain or other safety appliance would have prevented the accident.
