Tubs of ashes were being sent from the surface to a finished coal face to build an airway along it to prevent a gob fire. Three tubs had been lowered by the brake of a small motor stationed at the top of the hill 85 yards from the face. The first tub was uncoupled, and deceased was at the front end ready to twist it on the flat-sheet, when the other two moved on and pushed the first on rails below the flat-sheet. Deceased was crushed between the tub top under a low bar. He was released in a few minutes, but was dead. The motor minder had pegged the brake down. He had probably touched it unawares with his foot when passing, which released the peg and allowed the two tubs to move forward to the flat-sheet on a gradient 1 in 4.75.
