Inspection made and inquest attended. The deceased got in an empty tub in the endless rope downbrow and another person rode on the clip which he had loosened to slide on the haulage rope when it came to a pulley the latter was thrown off and the tub ran downbrow and deceased was fatally injured. The other person was convicted for Breach of Rules.
